Menlo College Student Insurance

The Menlo Student Injury and Sickness Insurance Plan is underwritten by United States Fire Insurance Company and managed by Summit America Insurance Services. If you don’t waive coverage, you will automatically be enrolled in the insurance plan and receive an ID card showing proof of coverage.

What It Covers

The Menlo College insurance policy covers sickness that starts or injury that occurs during the coverage period. The policy covers prescription drugs and provides coverage worldwide.

What Isn't Covered

  • Routine dental treatment
  • Eye exams (including glasses and contacts)
  • Hearing exams
  • Acne
  • Allergies
  • Family planning
  • Cosmetic surgery
  • Elective treatments
  • Surgery or injuries resulting from participation in intercollegiate, professional or organized sports competitions.

Dates of Coverage

You will have coverage beginning on [date], 2013 and ending [date], 2014, including holidays and vacations. If you graduate or withdraw from Menlo, the insurance will remain in effect until the end of the period for which you have paid the premium.

Deductible and Maximum Benefit

You must pay a $100 deductible per policy year. The maximum benefit per injury or sickness is $100,000 per policy year.

Doctors

Benefits are paid differently depending on whether you receive treatment from a provider that is “in network” or “out of network.” Use a provider in the First Health network to receive the highest benefit payments. Learn more about health care services near Menlo College.

Prescriptions

Prescription drug coverage is included. You will pay a co-pay of $15 on generic drugs and $30 on brand name drugs. You need to fill your prescriptions at a ScriptCare pharmacy. To find a pharmacy call (800)880-9988 or login to your account at www.scriptcare.com.

Travel Coverage

Because the Menlo College insurance plan provides worldwide coverage, travel assistance coverage is included through On Call International. They provide 24-hour services to help you access emergency assistance when you are traveling 100 or more miles from home.
