Insurance
Information
The
athletic injury insurance carried
at Menlo College Athletic Department
is a secondary or excess type
policy. All athletes are required
to possess a primary insurance
policy before being allowed to
participate in any intercollegiate
athletic activity. Upon injury,
the athlete’s primary insurance
is initially billed. After the
primary insurance has covered
its portion the claim is sent
to Menlo College’s secondary
insurance carrier for processing.
In the case that an injury is
incurred athletes must report
to the Menlo College Athletic
Training Facility prior to seeking
any other treatment, unless in
emergency situations. Athletes
seeking care without the approval
of the Menlo College Athletic
Training Staff will be held accountable
for these expenses. If an athlete
is injured but his or her insurance
policy has expired or was cancelled
the athlete is responsible for
paying the balance of medical
expenses. Athletes should also
possess a dental insurance plan.
Athletes requiring dental services
are bound by the same terms of
Menlo College’s secondary
insurance policy.
Note: If
a student-athlete is covered under
an HMO plan, he/she is required
to abide by the terms of the plan.
The limits
to the insurance policy are as
follows: It is an excess type
policy. The coverages are $50,000
for accidental death and dismemberment;
$75,000 maximum coverage for medical
expenses in each single claim/injury;
the deductible is $500.00 for
all sports. There is a 104 week
pay-out period. The student-athlete
is responsible for paying the
deductible.
Athletes
that do not have primary insurance
should contact Mark Fowler, Head
Athletic Trainer, at (650)543-3931
or mfowler@menlo.edu. To enroll

Insurance Policy please visit
